Ex Parte Application for Restoration of Former Name After Entry of Judgment and Order

This form allows someone to ask a court to change their name back to their former name after a divorce or legal separation has been finalized. A person who changed their name as part of a judgment would use this form to restore their previous name without having to go through a full new name-change court process.
