This form is used to officially request that a court proceed with finalizing an adoption and granting parental rights to the adoptive parents. Social workers, adoption agencies, attorneys, and prospective adoptive parents use it to formally ask the court to complete the adoption process and legally transfer custody and parental rights from birth parents or guardians to the adoptive family.
