This form is used to notify someone that they are required to appear in court for a domestic violence case. A person involved in a domestic violence situation—either the person seeking protection or the person being accused—would need this form to know when and where to show up for their court date.
This form must be printed and filled by hand, or opened in Adobe Acrobat.
