This form is used to confidentially assess and document whether a person has the mental or physical capacity to make decisions about their own care, finances, or property in a probate conservatorship case. Attorneys, conservators, and healthcare providers use it to present evidence to the court about a person's capacity when requesting to establish or continue a conservatorship.
