This form is used to document that a defendant in a family law case (like divorce or custody) has been officially notified about the lawsuit by having the court papers delivered to them. A process server, sheriff, or other authorized person who delivered the papers would complete and file this form to prove to the court that the defendant was properly notified.
This form must be printed and filled by hand, or opened in Adobe Acrobat.
