This form is used to officially approve a request from someone who cannot afford to pay court fees and costs associated with filing a case. People who have very low income or limited financial resources use this form to get a judge's permission to proceed with their court case without having to pay filing fees and other court expenses.
This form must be printed and filled by hand, or opened in Adobe Acrobat.
