This form allows a person to officially confirm that they are the biological or intended parent of a child in a stepparent adoption case involving a gestational surrogate. A stepparent, surrogate, or other party involved in the adoption uses this form to establish and document parentage during the legal process of adopting a child born through surrogacy.
