FL-341: Stipulation and Order Regarding Custody and Visitation

This form is used when parents agree on a custody and visitation arrangement for their children and want the court to officially approve and enforce their agreement. Either parent involved in a custody case would use this form to submit their agreed-upon plan to the judge instead of having the judge decide for them.
