This form allows a juvenile court to request that a young person's case be returned from adult criminal court back to juvenile court, and for the person to be placed back into foster care if needed. A district attorney, probation officer, or other court official uses this form to petition for this transfer when they believe it's in the young person's best interest to be handled as a juvenile rather than as an adult in the criminal system.
