MC-030: Declaration

The MC-030 is a written statement where someone describes facts they know about a case and swears under penalty of perjury that everything they wrote is true, used instead of testifying in court in person. Anyone involved in a court case—a party to the lawsuit, a witness, or someone with knowledge of relevant facts—would use this form to provide their account of events to the judge.
