FL-144: Stipulation and Waiver of Final Declaration of Disclosure
Fillable PDFJudicial Council of California
This form allows a married or registered domestic partner couple to skip the requirement to exchange detailed financial documents (called the Final Declaration of Disclosure) before their divorce or separation is finalized. Divorcing or separating couples use this form when both spouses agree they don't need to exchange complete financial information, typically because they've already shared their finances or have reached a settlement agreement.
